#1----the idle is hunting between 650 & 750 rpms
 
#2----After startup, the car idles by itself , but gets worse and worse unless I continue to add throttle, as the engine warms up.
 
#3----The car will not rev up to max rpm at any point in the warmup.  As a matter of fact, the
warmer it gets, the worse the sputtering and hesitation.  Almost like three or four plugs not firing.

Question 1: Is your idle relatively steady, or is the engine speed
surging with repeated small to moderate revs?

Question 2: Just to get this straight, when the vehicle is cold, it
idles, but lower than 750± RPMs? But then when it warms up, it dies
unless you keep your foot on the gas pedal? Or do you constantly have
to hold your foot on the gas pedal?

Question 3: Does the engine rev up normally, or does it hesitate or
sputter at all? In other words, if you press on the gas pedal, does it
go "vvvvrrrroooOOOOMMM M!!!", or does it make a sound like
"bbbrrrrrrrrrrrrRRR VVVVOOOOMMMM! !!"?
