Matt,
If you look here http://www.ohiodeloreans.com/Bauerle_Brackets.html (scroll all
the way to the bottom) you will see an accident directly related to the tabs.

Last year a tab broke on Rich Actui's car while he was driving at about 30mph.
Jerked the snot out of his back. If traveling at highway speeds it would have
been much worse (for both Rich & the car). 

I don't know if these qualify as "Catastrophic" but certainly directly related to
the tabs.
