Ryan - you were right on. I knew that nut was there but for some reason I
was not thinking it was tightened against the knob's nut (careful, I am
talking about the car now!). With a 13mm wrench on that nut, and a pipe
wrench on the knob (thanks Rob) I was able to get it off.

On the threads and inside the knob there were the remnants of some kind of
contact cement. It looks like the prev owner had tried to make sure it would
not come off.
