Absolutely vital. Without one you cannot register your car.

The C&E 386 is proof from UK Customs that all necessary duties have been
paid.

Assuming you imported your car as a runner, you should have received one
from customs, if not get in touch with the customs at your dock and find
out what's happened to it. Expect to get the run-around though :(
