Ben,
After reading your extensive report on the available exhaust systems for the D in the US, I only can compliment you on your thoroughness.
For your fellow D owners the European free flow exhaust system (yes, with lambda bung) is a bit more expensive.
Ed Uding's development is now on several European cars (also on my 6513) and I myself love it, even when making distances.
IMO this exhaust system is a little less loud than John's, but it sounds great. It brings approx. 25 BHP over a stock exhaust system.
So, when combined with a performance cam it could easily match or perhaps exceed the gain of the system you described.
Mind that most European D's do not use a cat and so have some 110 BHP on a dyno. Mine did 160.2 BHP, standard cam, no turbo's etc. I have a dyno report to support this. The only disadvantage while dynoing was that the car "stuttered" between 2200-2800 RPM, thus influencing the result which could have been higher according to the tester.
Once this stutter problem is solved I will have another dyno test performed to see the true results and will publish here.
