Dave S was right on the money.
I did have some debris on the evaporator screen that I vacuumed out
which I'm sure helped a bit, but the primary culprit was the foam
cover over the large rear hole in the box. The glue had probably dried
up and a large portion of the foam block was loose and flapping free
(exposing a large hole in the back of the box).
You have to be a bit of a monkey (long arms with a couple extra
joints) to get back there, but you can feel your way around from the
driver's foot well.
Thanks everyone for the tips and help with the airflow issue.
