Anyplace air can get in is a potential place for exhaust to get in.  You can 
attack this problem two ways - make sure your exhaust is complete, has 
no leaks, and is not obstructed.  Then make sure any of the possible 
ways that exhaust can get drawn back into the car are plugged - cable
passthroughs, fixed window seals (rear glass, mainly), bulkhead seals, 
etc.  Obviously, you have openings that are supposed to be there, like 
doors, windows, and fresh air vents, but they should seal well when closed
as well.
