The Poland entry was a victim of my new formatting system. The country
and state titles are supposed to all be headlines that generate the
table of contents. I'm making a bunch of little fixes as they are
brought to my attention and I'll post revised versions. The revision
number will be listed on page 2. New additions will still have to wait
for the next edition, which I expect will be this fall.
